Case Manager Position
The Case Manager is an integral team member, guiding families through our residential program and providing eviction prevention supports in the community. This organized, compassionate, and motivated person will assist families and individuals experiencing homelessness, to overcome obstacles, reach individual goals and secure and sustain housing.
Responsibilities:
- Working from a trauma-informed approach to guide families and individuals through barriers, primarily onsite in our Derry location, while providing onsite support in our Nashua location as well
- Assist with employment application, interview preparation, and career and educational development.
- Provide resources, referrals, and opportunities related to employment, financial education and budgeting, housing plans, and educational planning. Utilize organization’s resources and partnerships.
- Find housing for families by liaising with landlords and getting families ready as needed.
- Serve as an advocate for families with community service agencies.
- Complete intake assessments for prevention and diversion client referrals and work with the Director of Programs and Services to provide financial eviction prevention assistance
- Record detailed notes for all resident and client interactions and assist with grant reporting as applicable
- Flexible scheduling is necessary, involving evening meetings and program classes, as needed.
- Willingness to be part of the FPSNH team, and assist at fundraisers and special events, as needed.
Education, Experience, and Skills:
- The successful candidate is enthusiastic, positive, with sound judgment and boundary knowledge.
- Can take initiative, and help families achieve outcomes.
- Must have a sense of urgency, and creativity with a resourceful attitude.
- Advanced experience in a related field is required; related to housing and shelter, family services.
- Knowledge of, and ability to work with specialized populations and programs.
- Must be detail oriented, motivated, and able to prioritize
- Must be proficient in computer programs and demonstrated record of tracking outcomes, recording case notes.
- Bilingual candidates (Spanish/English) are encouraged to apply.
Salary and Benefits
• Full Time (40 hour/weekly) position with a competitive salary based on experience.
• Paid vacation time and holidays available
- Eligible for health insurance reimbursement, and mileage reimbursement for work-related car travel.
Work Environment, Schedule & Managing Success:
• The Case Manager reports to the Director of Prevention and Programs
• FPSNH is comprised of 10 Staff, 15 Member Board of Trustees, and 100 Volunteers.
